To be eligible for a kind act of service you must be between the ages of 2 1/2 years and 18. If you are the parent or legal guardian of a qualified recipient, please fill out the form below. The information you provide will be reviewed, and if accepted, you will be called by phone. Potential recipients may also refer themselves or be referred by a physician.
